BAY SPRINGS MIDDLE SCHOOL
STUDENT REGISTRATION
The following items are needed to enroll a student at Bay Springs Middle School:
- Birth Certificate
- Immunization Record Form 3231
- Ear, Eye and Dental Form 3300
- Social Security Card
- Proof of Residence
cable, gas, electric, water or phone bill
or rental/purchase agreement
- Withdrawal form or final report card
from previous school
- Custody Papers (if applicable)
All other records will be forwarded from the previous school.
